Proof of Vaccination against COVID 19 collection form.
On Sept 3rd KMHA posted an article informing our participants that Alliance Hockey and the OWHA have passed policies requiring eligible participants to be (double) fully vaccinated to participate in Hockey programming. At this time only Alliance Hockey has detailed their process. As soon as OWHA has shared information we will post to this form for you to submit.
Alliance Policy Statement: (Page 6) ALLIANCE Hockey will require proof of COVID-19 immunization for all vaccine eligible participants (12 years of age and above and unless exempted) including players, team officials, game officials, staff and volunteers in order to participate in ALLIANCE Hockey sanctioned programming. Eligible participants will require proof of having received their first dose by September 17, 2021 and be fully vaccinated by October 31, 2021.
All participants must be fully vaccinated for COVID-19 by October 31, 2021, which must include the two-week period required to be considered a complete series (second dose prior to October 17, 2021).
KMHA’s responsibility under this policy is to collect proof of vaccination and communicate eligibility for participation with our coaches. KMHA will not share medical information and coaches will be instructed they may not inform anyone else if a player is ineligible. KMHA will also communicate to the parents of players that have not yet submitted the correct documentation.
Ineligible participants will not be allowed in the programming area and are asked to refrain from attending. Should players wish a refund due to this policy they request one through the KMHA office at [email protected].
In order to submit the following steps must be completed.
- Retrieve you proof of vaccination. In compliance with Alliance policy the following forms are considered acceptable evidence of vaccination:
• A digital or physical Dose Administration Receipt
• Provincial Government issued verification passport
