Alliance Vaccine Policy Update

Good Day KMHA Family

On Sept 3rd KMHA posted an article informing our participants that Alliance Hockey and the OWHA have passed policies requiring eligible participants to be (double) fully vaccinated to participate in Hockey programming. KMHA still awaits details of the OWHA plans. We will move forward with a policy for our female programming once we hear from OWHA.  We have September 16th received confirmation that Alliance Hockey is continuing implementation of their Vaccination policy. As part of Alliance Hockey KMHA is required to follow all Alliance policies. As such KMHA has indicated we will follow the Alliance Vaccination policy for Alliance programming(Boys/Coed). Proof of participant vaccination will be required as follows.


Alliance Policy Statement: (Page 6) ALLIANCE Hockey will require proof of COVID-19 immunization for all vaccine eligible participants (12 years of age and above and unless exempted) including players, team officials, game officials, staff and volunteers in order to participate in ALLIANCE Hockey sanctioned programming. Eligible participants will require proof of having received their first dose by September 17, 2021 and be fully vaccinated by October 31, 2021.


All participants must be fully vaccinated for COVID-19 by October 31, 2021, which must include the two-week period required to be considered a complete series (second dose prior to October 17, 2021).


The timelines to complete this task are tight so please read how we need to proceed.


KMHA’s responsibility under this policy is to collect proof of vaccination and communicate eligibility for participation with our coaches. KMHA will not share medical information and coaches will be instructed they may not inform anyone else why a player is absent. KMHA will also communicate to the parents of players that have not yet submitted the correct documentation.  As the confirmation of this policy remaining after the government of Ontario’s announcement. KMHA will allow participants until 11:59pm September 20th to submit their documentation.  For those choosing not to participate due to this policy a request for refund can be made to [email protected]. Please provide your child’s name and date of birth on your cancellation request.  Any further questions or comments regarding the policy should be sent to the Alliance at ([email protected]) and not in the cancellation request.

Your role is to submit your proof of vaccination via one of the following on-line forms (files are stored on a protected and encrypted server and will be destroyed once no longer required):


Players please submit HERE.


Volunteers, and Staff please submit HERE.


In order to submit your documentation the following steps must be completed. 

  1. Retrieve you proof of vaccination. In compliance with Alliance policy the following forms are considered acceptable evidence of vaccination:

  A digital or physical Dose Administration Receipt

  Provincial Government issued verification passport
