This form is to be completed with all required details to request a cheque payment for a tournament for your team. Also, to be used to request reimbursement for a tournament payment made by the coach/volunteer in advance of the team account being setup and monies collected. Cheques will take about 7-10 business days to be issued and signed. We will send an email when the cheque is ready for pickup in the KMHA office.

All information to be completed FULLY for your cheque to be completed. Don't forget to complete your travel permit as well.

PLEASE COMPLETE THE CHEQUE INFORMATION CAREFULLY AS THIS IS THE INFORMATION USED WHEN THE CHEQUE IS ISSUED.

REISSUING OF CHEQUES FOR IMPROPER INFORMATION ENTERED ON THIS FORM MAY RESULT IN A CHARGE TO THE TEAM.
