IMPORTANT UPDATE - EVALUATION
SELF SCREENING
The "OHF Health Screening Questionnaire" document must be reviewed, signed and submitted prior to the scheduled ice time. Families are to complete the screening verbally with their child to ensure they have no COVID symptoms and provided the child is symptom free, can bring a signed copy to evaluations. Any players showing symptoms, MUST NOT attend a team event and will be responsible to notify the Coach directly. There will be a few copies available to fill out and sign tomorrow at "check in" if you are unable to fill out in advance.
